Community Health Network Foundation

At Community Health Network, health and safety remain our top priorities. Community Health Network Foundation has taken several precautionary measures, including working remotely, with the intent of best protecting and serving our patients and communities, as well as our caregivers and also donors like you.

As Foundation team members will not be onsite to process gifts, the most efficient way to support Community's caregivers and patients at this time is through online giving. Our secure donation page is accessible through the "Donate" button below. Gifts made in response to COVID-19 can help provide support or financial assistance for Community employees facing sudden and severe hardship or support patients in financial need. Giving Tuesday is December 1

December 1 is Giving Tuesday, a global generosity movement unleashing the power of people and organizations to transform their communities and the world. On this day, we invite you to give in support of Community Health Network’s patient assistance fund, which provides immediate assistance to patients in Community primary care settings across Central Indiana, helping them access basic needs like medicine, food and transportation while bridging them to longer-term solutions to best address their health-related social needs. Your gift of $25 or more can help to change the day of our patients. 

Right now, there's nothing more important than your health. Across Community Health Network, we're committed to helping our patients break down the barriers to care - including the financial aspects. We aim to make the financial burdens less worrisome so our patients can focus more on their health. 

With the support of donors like you, patient assistance programs play a key role in providing financial relief to patients. Whether it is helping a family put food on the table or ensuring a patient gets home safely from an appointment, your generosity directly impacts men and women in our communities, helping them access basic needs and letting them know we stand by them, united against the challenges they face. 

In this year unlike any other, patient assistance programs are as important as ever. We hope that, after watching this video, you can see how, together, we can enhance the health and well-being of more individuals and families across Central Indiana.

Our Work

Since 1976, Community Health Network Foundation has worked to raise and steward financial support for Community Health Network, a non-profit health system providing a full continuum of care through more than 200 sites and hundreds of physicians throughout Central Indiana.

Our mission? To support Community Health Network’s commitment to improve the health of the communities we serve.

All operating expenses of the Foundation are covered by Community Health Network, while generous sponsors fund the overhead costs of our annual fundraising event The Giving Gig. Thanks to this support, 100 percent of all donations made to Community Health Network Foundation benefit patients and help to fulfill the mission of Community Health Network.
