Advanced Practice Provider Students

We offer high-quality clinical education experiences for Advanced Practice Provider (NP, PA, CNM, SRNA, CNS) students from affiliated schools who meet specific criteria. Available practice settings for qualified APP students to complete their clinical education include family medicine, internal medicine, specialty care, urgent care, women’s health, and behavioral health.

Advanced Practice Provider Student Clinical Rotations

Thank you for your interest in Community Health Network as a site for a portion of your clinical education. We are happy to evaluate your request for placement. Our hope is that students who come to Community have an exceptional educational experience. In order for us to provide that experience, please review this information carefully and follow the steps below.

All NP and CNM students must submit their application for the upcoming rotation(s) prior to the deadline(s) below. Student notification will take place on or before the date listed.

*PA, SRNA, and CNS students can apply at any time. Applications for NP and CNM students will not be accepted after the deadline has passed.
**Rotation dates are defined by the APP placement team.

Note: Due to the large number of requests and limited preceptors available, Community Health Network has the following restrictions in place:

  • NP and CNM Students: Community Health Network prioritizes placement of our caregivers and only accepts students from universities with which we have a fully-executed affiliation agreement on file. If an affiliation agreement is not on file, obtaining one can take several months, if approved.
  • PA Students: Community Health Network only places students from PA programs that are in Indiana.

Application Instructions

All students who meet the criteria above and are seeking placement or intend to engage in a clinical rotation at Community must complete our online clinical placement application.
