Advanced Practice Provider Student Clinical Rotations

Thank you for your interest in Community Health Network as a site for a portion of your clinical education. We are happy to evaluate your request for placement. Our hope is that students who come to Community have an exceptional educational experience.

All NP and CNM students must submit their application for the upcoming rotation(s) prior to the deadline(s) below. Student notification will take place on or before the date listed.

*PA, SRNA, and CNS students can apply at any time. Applications for NP and CNM students will not be accepted after the deadline has passed.
**Rotation dates are defined by the APP placement team.

Note: Completing either form does not guarantee placement. Due to the large number of requests and limited preceptors available, Community Health Network has the following restrictions in place:

  • NP and CNM Students: Community Health Network prioritizes placement of our employees and only accepts students from universities with which we have a fully-executed affiliation agreement on file. If an affiliation agreement is not on file, obtaining one can take several months, if approved.
  • PA Students: Community Health Network only places students from PA programs that are in Indiana.
