Care Coordinator - Pando
Provides collaborative planning, referral and monitoring of treatment and support services to adults with severe mental illness, persons with chronic addictions, or children with severe emotional disturbances and their families.
2 years of experience in a position requiring training and case management preferred.
Bachelor's degree in Psychology, Social Work, or related field required.
Valid Indiana driver's license and proof of current auto insurance; driving record that adheres to CHI policy.
