M-F 8-5 or 4- 10 hour shifts.

Highlights
Community Health Network is seeking a team-oriented & skilled Nurse Practitioner with transplant or VAD experience.
- Fully integrated advanced program offering all services with the exception of transplantation
- Chest pain accreditation with 24/7 STEMI coverage, VAD accreditation, structural heart program, impella program, heart failure program, cardio-oncology, pulmonary hypertension
- Expanding VAD program
- Electronic Medical Record system (EPIC)

Licensure & Certification
- Must have current licensure as a Registered Nurse in the State of Indiana
- Must have current licensure as an Advanced Practice Nurse
- Must successfully be credentialed by the Network Medical Staff Office
Desired requirements
• Minimum of 3 Years of Transplant or VAD experience
• Excellent communication skills fostering collaboration with all members of care team
