Thank you for your interest in the Community Hospital East Family Medicine Residency program at Community Health Network. Our innovative program trains family physicians to become outstanding leaders in the delivery of primary care, with an emphasis on practice in the Patient Centered Medical Home. Our residency is committed to cultivating and maintaining a diverse and inclusive workspace.
Our unique longitudinal curriculum is designed around the concept that adult learners incorporate information best by revisiting important topics over time. While emphasizing time in the Patient Centered Medical Home, we also hope that each resident who comes to our program can carve out an individualized educational experience that will prepare them for whatever setting they ultimately enter upon graduation - full-spectrum family medicine, fellowship, hospital work or outpatient practice.
As a resident in our program, you will train in a primary care-focused community health network, an environment we feel is ideal for instruction in family medicine. We emphasize interdisciplinary teamwork and process improvement in our residents’ education. Our program carries a long track record of excellence as evidenced by our continuous accreditation by the ACGME, our graduates’ success on ABFM certification exams, and the support from our health network for family medicine education for over 40 years.
We care for many underserved patients in our offices and hospital, which prompted us to start an underserved care track in 2016. Our hospital on the east side of Indianapolis opened a new patient care tower in February 2019 to continue its history of being a strong pillar in our neighborhood.
Although our website contains a lot of information, it cannot tell you who we really are. Our residents, faculty and staff are the heart of our training program and we’d love to have the opportunity to have you learn more about us. We have created a friendly and supportive team-based approach to patient care and learning, and we are committed to diversity and inclusion. We are also pleased to announce Sports Medicine and Addictions Fellowships were approved and will begin in 2021. Please consider joining our team!
