Care Beyond the Hospital Walls

Our mission of enhancing health and well-being is a whole lot more than simply providing healthcare services within the walls of the hospital. Because we know that long-lasting health doesn’t happen from a single visit to the doctor or a trip to the emergency room. This is why we provide care that goes beyond our facilities and into the community.

We also know that many people in Central Indiana have obstacles standing between them and care. Housing instability, food insecurity, utility limitations, interpersonal violence and transportation are a few of the factors that can affect access to care and the lifelong health of our neighbors, friends and family. Our team of Community Health Advocates live in the neighborhoods that they serve and help identify the issues Hoosiers are facing and connect them to the resources they need.

Community Health Advocates and our Community Connections resources help connect between 1,200 and 1,500 people to services for specific needs each month, playing a critical role in solving for social determinants of health outside of the hospital.
