Vynca and Community Health Network partner to digitize advance care planning

Collaboration Ensures Patients’ Wishes are Heard and Respected

PALO ALTO, CA and INDIANAPOLIS, IN – June 15, 2021—Vynca, the national leader in advance care planning and serious illness care, and Indianapolis-based Community Health Network (Community), today announced a partnership that will enable patients and the Community care team members to digitally create and share advance care planning documents.

Vynca’s technology will seamlessly integrate in to Community’s Electronic Health Record system. This will allow providers and staff throughout Community’s vast network to have easy and quick access to detailed information within the clinical workflow. Patients will also have access to an easy-to-use digital portal where they can securely review, create and sign documents expressing their end-of-life wishes.

“We will be working closely with Vynca to improve our current advance care planning process flow, documentation and training for Community’s staff in all areas of care, also to increase patients and families awareness of the importance of advance care planning conversations and at the same time improve the trust and confidence our team members have in stored advance care planning documents in our system,” said Dr. George Hoyek, Physician Executive for Post-Acute Care at Community Health Network. “We are looking forward to this opportunity because we know by being proactive in advance care planning we can ensure patient’s wishes are heard and respected.”

Vynca currently houses active advance care plans for almost one million individuals across all 50 states. Partnering with over 150 hospitals, as well as health plans, ACOs, medical groups and state registries across 18 states, these organizations are realizing an impact in terms of higher quality of care at the end-of-life and the avoidance of unwanted health care interventions.
