Community Health Network and CHI Health at Home advancing health care together

INDIANAPOLIS, IN – Community Health Network and CHI Health at Home, one of the largest home health care systems in the country, have signed an agreement to form a new joint-venture partnership to provide home-based health care solutions throughout Central Indiana.

Community Health Network, a leader in providing convenient access to exceptional health care services where and when patients need them, has formed this new partnership with CHI Health at Home to enhance their services, assure a stronger and more versatile organization, and to expand its home care and home infusion service offerings and value-based solutions.

“We’ve pursued this partnership on behalf of Community Home Health Services because it is an opportunity for Community Health Network to partner with another industry expert to find new ways to grow in all areas of service, infuse strong population health strategies, and maximize new technology to enhance the experience for our patients and our caregivers,” said Community Health Network Chief Financial Officer Kyle Fisher.

Community Home Health Services has been providing home health care to residents of Central Indiana for more than 55 years. From its beginning as a volunteer home health agency to today, they are one of the largest agencies in Indiana. Their growth comes from the quality and breadth of their home health services provided by skilled caregivers that are dedicated to quality care for all home care and hospice patients.

This alignment is in step with a growing trend where health care organizations across the country are aligning and building strong partnerships to maximize access to resources and capitalize on best practices to help them evolve.  Organizations seeking to succeed in a rapidly evolving marketplace, with payment systems rewarding value more than volume, and a shrinking specialized workforce are strategically aligning in markets where scale and diversification can positively impact growth and expansion.

Starting August 12, Community Health at Home will join the System to provide quality, compassionate home care to the existing patients while integrating over 200 home health care associates and caregivers to CHI Health at Home’s state of the art electronic medical record system, Homecare Homebase, and their successful chronic disease management programs that have helped position 79% of CHI Health at Home’s home care locations to be rated as a 4 Star or higher in a pursuit of clinical excellence.

Later in the year, Community Health at Home, dba AmeriMed, Inc will begin providing home infusion services throughout Central Indiana. AmeriMed is a leader in the industry and currently provides service in Ohio and Kentucky with partners in Iowa and Nebraska. Using CPR+ they offer specialized individual and coordinated care with local partners.
