Community Impact Award

The Community Impact Award is presented to Community Health Network physicians and APPs for their positive impact through service to their community, service to their profession, and humanitarian work. We want to recognize these individuals not only for their achievements, but also for the inspiration they provide others.

Any CHNw employee may nominate a physician or APP who has made made an outstanding contribution through active service in the medical community through any of the following activities:

• Upholding the practice of medicine through advocacy of national health care policies at the local, state, or federal levels on behalf of his/her profession and patients.
• Served in a leadership role in local, state, or national medical or professional societies.
• Has had a significant impact on their community, through clinical services, teaching, research, volunteer work, or other community involvement continually advocating for the best health of those served.
