Community Hospice prides itself on putting patients and their loved ones first. We provide expert level pain and symptom management care by addressing the physical, spiritual, social and emotional needs of individuals facing the end of life, while also supporting their loved ones. Our focus is on the quality of life remaining and for each individual to live with dignity, respect and as much independence as possible. Our care team is made up of physicians, nurse practitioners, registered nurses, social workers, music therapists, hospice aides, chaplains and volunteers. The team brings support and comfort to patients wherever they wish to receive their care. We support patients in the following counties: Hamilton, Hancock, Johnson, Madison and Marion.

Pre-Hospice Consultation

If you are unsure if hospice care is right for you or your loved one, a pre-hospice consultation may be helpful. For a consultation, call us at 317-621-5900. The patient, patient’s physician, family member, nurse, social worker or healthcare provider may request a consultation.

A pre-hospice care consultation may consist of the following services:

  • Conversation with patient and family covering goals of care
  • Clinical history review
  • Evaluate qualifications regarding hospice Medicare benefit
  • Answer any questions patient or family has regarding hospice or their serious illness
  • Discussion of patient’s wishes related to advanced care planning
  • Review of serious illness symptoms and best practices related to medication management

What is hospice?

Hospice is a specialized care that focuses on the patient’s wishes, desires and comfort as well as providing family support when a patient no longer responds to curative measures.

Where can I receive hospice services?

Hospice patients are cared for in their homes, assisted living facilities, nursing homes and inpatient hospital settings…wherever the patient is most comfortable and considers to be home.

How does hospice care work?

Typically, a family member serves as the primary caregiver and, when appropriate, helps make decisions for the seriously ill individual. Members of the hospice staff make regular visits to assess the patient and provide additional care or other services. The hospice team develops a care plan that meets each patient’s individual needs for pain management and symptom control. Support for family caregivers is also assessed throughout the care period. While the range of services provided will vary depending on each individual situation and the specific needs, hospice staff are available by phone 24 hours a day/7 days a week.

What services are provided?

Our interdisciplinary hospice team consists of a hospice medical director, registered nurses, certified home health aides, social workers, chaplains, music therapist and volunteers. The goal is to manage symptoms and pain, as well as assist the patient and family with emotional and spiritual needs. We provide medical equipment, supplies, and any medicines related to the hospice diagnosis, needed by the patient. Our team coaches the family on how to care for the patient. In addition, our grief specialist provides bereavement counseling to family after the loved one passes.

How do I know if I qualify?

Hospice care is for any person who has a life limiting illness. Most reimbursement sources require a prognosis of 6 months or less if the illness runs its normal course. Hospice considers the patient and the family together on their plan of care.

How do you pay for service?

Coverage is provided by Medicare, Medicaid and most private insurance companies. We will work with your insurance company to file all necessary paperwork.

Hospice Care Team Roles

Meet the Hospice Providers

Physician/Nurse Practitioner - The physician or nurse practitioner, assumes responsibility for the medical component of the hospice journey. They will develop a plan of care with the patient, family, and the patient’s physician to best meet the patient’s wishes.

Registered Nurse - Our registered nurses are experienced in end-of-life care and are on call 24/7 in coordination with the patient’s physician. They provide disease education, along with pain and symptom relief.

Social Worker - Master's-level social workers specialized in counseling. They can arrange services or community resources and work to reduce stress and anxiety in the patient and caregiver.

Hospice Aide - Hospice aides assist with personal care needs such as bathing.

Music Therapist - Music therapists are credentialed professionals trained to use music to help meet the care goals of patients and families.

Chaplain - Our non-denominational chaplains provide spiritual support for patients and families during the hospice journey.

Grief Counselor - Our certified grief counselors provide 13 months of bereavement support for loved ones through individual counseling, support groups, memorial events and more.

Volunteers - Volunteers provide a crucial layer of support to patients and families. They may assist the caregiver by providing companionship, socialization or even help with errands.
