Sunflower Award Nomination Form

The Sunflower Award honors non-licensed caregivers who provide outstanding direct patient care and service to our patients and their families. Eligible job categories include medical assistants, patient care technicians, certified surgical techs, student externs, OR assistants and ED techs.

The Sunflower Award is available for caregivers at the following locations:

Community Hospital Anderson
Community Hospital East
Community Howard Regional Health
Physician practice offices

Anyone can nominate a caregiver for a Sunflower Award - including patients, families, coworkers, or physicians.

Nominee Information

This caregiver’s technical skills and especially his/her compassionate care exemplify the kind of caregiver that our patients, their families, and our staff recognize as an outstanding role model.
