As of July 1, 2016, expanded health and wellness center hours are available at two convenient locations. Medical Plan Members are eligible to schedule appointments at either Clark-Pleasant or Greenwood School’s Health & Wellness Centers.

Medications and refills
Beginning July 1, 2016, select generic medications and refill services will be available at the Clark-Pleasant Health & Wellness Center and Greenwood Schools Health & Wellness Center AT NO COST*.
*To qualify for free approved generic medications and refills you must: 1) Be a patient at either of the Clark-Pleasant or Greenwood School’s Health & Wellness Centers AND 2) Being treated for conditions requiring medications by one of the providers at either of the Clark-Pleasant or Greenwood School’s Health & Wellness Centers.
Cynthia Ebini, MD
Dr. Ebini was born in Cameroon, moved to the United States after high school, and attended the University of Illinois, Springfield where she studied clinical laboratory sciences. Dr. Ebini worked for a few years in the clinical laboratory field while studying for her master's degree in health care administration. After completing this program, she completed an administrative fellowship at the VA in Washington, D.C. After her fellowship, Dr. Ebini attended medical school at Ross University School of Medicine, Dominica. She completed residency training in family medicine at Indiana University in 2010 and worked as a family medicine physician and medical director at Citizens Health Center in Indianapolis. Dr. Ebini has worked for Community Health Network as a physician since 2013.
Patricia English, MD
Dr. English graduated in 1989 from IUPUI with a B.S. in Chemistry. She attended IU Medical School, graduating in 1993, and went on to do her family practice residency at St. Francis Hospital, finishing in 1996. Dr. English has been a family practitioner on the south side of Indianapolis since then. She joined Community Physician Network in December of 2011 and transitioned to MedCheck Greenwood in January 2016. Dr. English is married to Jeffrey, a middle school math teacher; they have four children. In her spare time, Dr. English enjoys reading, baking and playing with their puppy, Rico.
Soheir E. Ghattas, MD
Dr. Ghattas was born and raised in Sudan, North East Africa. She graduated medical school at Khartoum University in 1980. She practiced medicine in Khartoum for eight years. She moved to the United States in 1988 and worked in a family practice in California for five years. In 1993, she completed her family practice residency at IU Medical Center. She has worked for various medical networks, urgent care centers, primary care groups and onsite primary care clinics as a provider and medical director in the Indianapolis area. She joined Community Health Network in 2012. Dr. Ghattas is married and has one son. She enjoys music, traveling and visiting new places.
Alissa Cohoat, NP
Alissa Cohoat is a family nurse practitioner who grew up in South Bend, Indiana. She earned her bachelor of science in nursing from Saint Mary's College and her master's from Indiana University. She has an extensive background in women's health and pediatrics, but loves caring for people of all ages. In her spare time, she enjoys exercising, cooking, and spending time with her husband and three daughters.
Amber Fair, FNP
Amber Fair is an AANP board-certified family nurse practitioner. She earned her bachelor of arts in biology from Franklin College, a bachelor of science in nursing from IU School of Nursing and her master's degree from the University of Indianapolis. She has an extensive background as a nurse in emergency medicine and is comfortable caring for people of all ages. Amber is an active member of her church and loves spending time with her husband and three daughters.
Nissa Watkinson, NP
Nissa Watkinson is an AANP board certified family nurse practitioner. She earned her bachelor of arts in political science and sociology from Indiana University, bachelor of science in nursing from Indiana University, and her master's degree from the University of Indianapolis. She worked for five years as a nurse in the Intensive Care Unit at Community Hospital East. Nissa enjoys spending time with her husband, three young children, and her large extended family near Greensburg, Indiana. She enjoys caring for patients of all ages. In her spare time, Nissa enjoys hiking and traveling.

Ashley Martin is the health coach at the Greenwood Schools Health & Wellness Center on Thursdays from 2 to 6 p.m.
Ashley has worked for Community Health Network since 2013. She has a bachelor of science degree in physical education with an emphasis in exercise science. Ashley is an ACE certified health coach, a Certified Intrinsic Coach®, a facilitator for the American Lung Association's Freedom from Smoking program, licensed Zumba instructor, and is a certified AFAA group exercise instructor. In her spare time, Ashley enjoys playing volleyball, cycling, walking her dogs, and baking or cooking.
