Christine Jung, DO

Dr. Jung is originally from southern California. She completed her undergraduate education at University of California, Irvine with a bachelor's in information and computer science. Dr. Jung worked as a computer programmer for about two years before her husband, Dr. Peter Choi, convinced her to pursue medicine. She attended Touro University of Nevada College of Osteopathic Medicine before joining the Community Westview Family Medicine program.

Jeffrey Klak, DO

My goal is to provide comprehensive and compassionate medical care. I believe in patient education, preventive care, and in-depth education and treatment. I strive to actively involve my patients in their medical care and decision-making process to promote improved quality of life, health and wellness. It is my passion to provide comprehensive, goal-oriented primary care.

I obtained my undergraduate degree on full scholarship as an Evans Scholar at Indiana University in Bloomington with a major in biology. I went on to graduate from DeBusk College of Osteopathic Medicine in Harrogate, Tennessee.

I was born and raised in Texas where I did my undergraduate and master’s degrees at Texas Tech University. I then attended medical school at Arizona College of Osteopathic Medicine in Phoenix. I chose Community Westview’s Family Medicine Residency Program for multiple reasons, including the wonderful people that are directly and indirectly associated with the program, the clinic at Speedway Family Physicians, and the great physician support at Community Health Network.

Dr. Peter Choi grew up in Diamond Bar, CA. He majored in neurobiology at University of California, Berkeley and obtained a master's degree in physiology and biophysics at Georgetown University in Washington, D.C. He graduated from Western University of Health Sciences College of Osteopathic Medicine in Pomona, CA.

Dr. Melton joined our faculty team after graduating from the Community Westview Osteopathic Family Medicine residency. She is originally from Central Kentucky and went to Georgetown College where she received a bachelor of science in biology. She then attended the University of Pikeville Kentucky College Of Osteopathic Medicine where she earned her doctorate degree and completed an undergraduate fellowship in osteopathic manipulative medicine.

Martha Nielsen is originally from Upper Black Eddy, Pennsylvania. She received a bachelor’s degree in biology from Ursinus College in Collegeville, PA before attending Lincoln Memorial University-DeBusk College of Osteopathic Medicine in Harrogate, Tennessee.

Joseph "JT" Silvers, DO

JT is from the small town of Monticello, Kentucky and graduated summa cum laude with a bachelor’s of science degree in biology from Campbellsville University in central Kentucky. He then went to medical school at the University of Pikeville Kentucky College of Osteopathic Medicine, where he also completed a teaching fellowship in osteopathic manipulative medicine.

Dawn grew up on a small farm outside of the Southeastern Indiana town of Madison. She received a bachelor of arts degree in psychology from Butler University in 2005, and went on to study for a master's degree in public health at the University of Kentucky until beginning medical school in 2007. She graduated from medical school at University of Pikeville Kentucky College of Osteopathic Medicine in 2011 before joining the residency.

Meredith Potrzebowski, DO

Dr. Potrzebowski grew up in Marion, Indiana and graduated from Purdue University in 2007 with a degree in health sciences. She completed medical school at Lincoln Memorial University-DeBusk College of Osteopathic Medicine in Harrogate, Tennessee. During residency, she completed additional training in cranial manipulation and began to learn acupuncture.

Dr. Sarah Lynley Rader is a native of Central Indiana. She obtained a B.A. in Political Science from Indiana University, and earned her medical degree from Kansas City University of Medicine and Biosciences. After completing her internship at the University of Colorado Medical Center in Denver, Dr. Rader returned to Indiana to continue her training through the Community Westview Family Medicine Residency.
