Our Touchpoint Senior Living practice consists of experienced physicians and nurse practitioners, dedicated to serving the geriatric population in post-acute and long-term care settings. The physician-led provider team works closely with the care teams at partner skilled nursing facilities and various senior living care settings to achieve the best possible outcomes for elderly patients.

Abdulateef Aregbe, MD
Abdulateef Aregbe, MD, attended the prestigious College of Medicine of the University of Lagos, Nigeria where he graduated in 1997 at the top of his class. He completed his internal medicine residency training at the Woodhull Medical Center in Brooklyn, New York between 2006 and 2009. After residency, he moved to Indiana to complete further training in clinical pharmacology at the Indiana University School of Medicine (2012). He is currently a member of the Community Pulmonary and Critical Care Medicine group at Community Health Network where he works as a hospitalist at Community Hospital North.
Dr. Aregbe joined Touchpoint Senior Living in 2016 and brings his expertise in pulmonary and critical care to patients in post-acute skilled nursing facility locations.
Dr. Aregbe is married with four children. He enjoys traveling and soccer.
Tammy Durham-Boring, DO
Dr. Tammy Durham-Boring earned her undergraduate degree in biological sciences from Washington and Jefferson College in Washington, Pennsylvania. She then earned her medical degree at the University of Osteopathic Medicine and Health Sciences in Des Moines, Iowa. She completed her internship and residency training in internal medicine at Des Moines General Hospital in Des Moines, Iowa. During her time there she was chief resident in internal medicine in her last year of residency and earned an excellence in internal medicine honorable distinction award.
Dr. Durham-Boring is board certified in internal medicine and board certified in hospice and palliative care medicine. before joining Touchpoint Senior Living she had an inpatient hospital and outpatient ambulatory practice in internal medicine at Community Westview Hospital for 14 years. She is involved in caring for the aged population and has been a provider at multiple extended care facilities in Indiana for more than 12 years. She is also a certified nursing home medical director through AMDA. She is passionate about providing care for those with terminal or progressive chronic ailments.
Dr. Durham-Boring is married and has a daughter and multiple step-children. She enjoys reading, cross-stitching and spending time with family and friends.
Georges El Hoyek, MD
Georges El Hoyek, MD, received his doctorate degree from Lebanese University School of Medicine. He completed an internal medicine residency at St. George Medical Center, a family medicine internship/residency at University of North Dakota, and a geriatric fellowship at University of Nevada School of Medicine. He is a certified medical director and most recently served as a hospitalist at Trinity Hospital.
He is a member of AMA, AGS, AMDA, AAFP, and Alpha Omega Alpha. He has a special interest in geriatric syndromes (frailty, fall, gait impairment), dementia/delirium/depression and geriatric acute/post-acute/long term care. He also has an interest in development and implementation of geriatric models of care.
He enjoys skiing, golfing, tennis and travel.
Radharani Eluri, MD
Dr. Radharani Eluri earned her medical degree from Gandhi Medical College in Hyderabad, India and served her residency at York Hospital in Pennsylvania. She completed The Banner Family Medicine Geriatric Fellowship Program in Arizona.
She is a Diplomat of The American Board of Family Medicine and a member of the American Geriatrics Society, American Medical Directors Association and the American Academy of Family Physicians.
Prior to joining Community Touchpoint, Dr. Eluri was a geriatric attending physician with Virtua Medical Group in New Jersey.
John Reece, MD
Dr. John Reece is the physician lead for post-acute services at Community Health Network. He provides executive leadership to aid in the development of an integrated geriatric practice. In this role, Dr. Reece also provides oversight for Community’s extended care physician practice, Touchpoint Senior Living, which provides care in 13 facilities in the greater Indianapolis area. He has provided care for patients at the Westminster Assisted Living clinic since September 2014 and was a family practice physician in Delaware County for 24 years.
A graduate of the Indiana University School of Medicine, Dr. Reece completed his family practice residency at Ball Memorial and is a diplomat with the American Board of Family Practice.
He resides in Noblesville with his family.
John D. Riley, MD, MPH, CMD
Dr. John Riley earned his medical degree at the University of South Florida, served his residency in internal medicine at East Carolina University and completed a fellowship at Indiana University in general medicine. Dr. Riley has been recognized as a Certified Medical Director in Long Term Care by the Board of Directors of the American Medical Directors Certification Program. He is board-certified in internal medicine and a certified home health care physician.
Prior to joining Touchpoint Senior Living, Dr. Riley spent 10 years in a house call and long-term care driven medical practice. He enjoys working with geriatric populations, and his special interests include dementia, hospice and palliative care.
Dr. Riley lives in Fishers with his wife, Dr. Treep, and his two sons.
Tamara Brown, FNP-C
Tamara Brown is a certified family nurse practitioner through AANP. Tamara received her associate degree in nursing from IU and both bachelor’s and graduate degrees from Indiana Wesleyan. Tamara has been working with the geriatric program since receiving her family nurse practitioner certification. Tamara loves working with the geriatric population and tries to make a positive impact on the patients she encounters. Previously, Tamara worked in the ICU for many years, which helped with her transition to working with the geriatric population.
Tamara is married to Robert and they share seven children together. She enjoys her family and her grandchildren. Traveling and a recent move keep her busy. The most unforgettable times were relaxing at the ocean and traveling to Hawaii, a place she hopes to return to one day.
Cheryl Ditzler, NP
Cheryl received a bachelor’s of science degree in nursing from Indiana University School of Nursing and a master’s degree in nursing from Vanderbilt University. She has more than 30 years of nursing experience and most recently worked in behavioral health as an APRN/BC. She joins our Senior Living team to provide behavioral health support for our patients.
She enjoys Pilates and yoga, cycling, gardening, and spending time with her nieces and nephews.
Megan Linden, FNP
Megan Linden is a board certified family nurse practitioner through the American Association of Nurse Practitioners. She is licensed by the state of Indiana to provide care to individuals of all ages. She received her bachelor’s degree in the science of nursing from Indiana University Purdue University of Indianapolis, and her master’s degree in the science of nursing from Indiana Wesleyan University. Prior to joining the Touchpoint team, Megan worked for Community Home Health and in a long term care facility as a registered nurse where she discovered her love of geriatrics.
Megan is married to Maurice, and they reside in Indianapolis. She has one son along with multiple step children. Her hobbies include spending time outside, walking the family dog, and spending time with friends and family.
Sarah Minges, MSW
Sarah is a master’s level social worker who earned her bachelor of arts in psychology from Indiana University and later received her masters of social work through IUPUI in 2006. Sarah began her career working in international adoption serving as a case worker to assist families throughout the complexities of adoption process.
Sarah joined Community Health Network in 2005 as an inpatient case manager at Community Hospital North. Eight years later, she joined the Touchpoint Senior Living team in 2013. Sarah is passionate about working with geriatrics and prides herself on her ability to advocate and support patients and families during a medical crisis.
Sarah and her husband, Will, live in Fishers with their son and daughter. She enjoys spending as much time as possible with her two children, cooking, and spending weekend summers at her family’s lake cabin.
Masita Poray, MSN, AGNP-C
Masita Poray is an adult-gerontology primary care nurse practitioner, certified through the American Association of Nurse Practitioners. She earned her masters of science in nursing from the University of Cincinnati in 2016. Masita has a bachelor’s degree in nursing and organizational communication from Ball State University. Her nursing background included working as a float nurse inpatient and outpatient and covering house supervisor needs. Masita has also worked as a travel nurse.
Masita was introduced to the geriatric population as a certified nursing assistant in high school. She has worked for Community Health Network for seven years and is excited to continue her career as an advanced practice nurse with Touchpoint Senior Living.
Masita resides in Noblesville with her husband. She enjoys traveling with friends, volunteering, and spending time with family.
Nicole Sandberg, BSW
Nicole is a social worker who earned her bachelor of social work from Ball State University in 2010. Nicole has spent her social work career in long-term care facilities, including five years as a social services director prior to joining the Touchpoint team in September of 2016.
Nicole and her husband, David, live in Fishers with their son. She enjoys reading, cooking and spending time with her family, including her dog and two cats.
Tiffini Smith, NP
Tiffini Smith is a board-certified adult-gerontology nurse practitioner through the American Association of Nurse Practitioners. She earned her associates degree in nursing from Ivy Tech, her bachelor’s degree in nursing from Indiana University-Purdue University in Columbus, and her master’s degree in nursing/AGNP from Ball State. She has dedicated her nursing career to the treatment of the aging adult. She is a member of AANP. Tiffini has professional interests in mental health, geriatrics and addiction treatment and recovery. Prior to joining Touchpoint, Tiffini spent more than eight years working in skilled nursing rehab.
Tiffini is originally from Baltimore, Maryland and is married to Jim. They have three grown sons between them. They also have a Maltese who tags along on many of their adventures. She enjoys bowling, nature hikes, growing her own fresh herbs, and taking her dog to visit the geriatric population.
Virginia E. Spears, MSN, FNP-C
Virginia (Ginny) Spears earned her masters of science in nursing from Indiana University and has been a nurse practitioner for the last 12 years, joining Community Health Network in 2006. She works with Touchpoint Senior Living patients on the east side of Indianapolis. During her 28 years in nursing she has worked in many areas, including surgery, open heart recovery, post-anesthesia care, peri-operative care, emergency nursing, dialysis, intensive care, critical care, psyche and in-patient detox. Education has long been a focus for Spears who developed a diabetic education program for St. Vincent Heart Center and has taught numerous classes for Ivy Tech Community College of Indiana and Indiana University. She is a member of Sigma Theta Tau, the American Academy of Nurse Practitioners and numerous other professional organizations. She is certified to teach Basic Life Support.
Spears and her husband Bob have been married for more than 40 years and have two grown sons. She enjoys spending time with family and friends, trips to the beach, and hiking, golf and tennis.
Kara Tullos, MSW, LSW
Kara is a social worker who earned her bachelor's and master's degrees in social work through IUPUI. Kara’s background is in mental health services where she worked for three years as a care coordinator serving older adults who were diagnosed with a severe mental illness. Kara joined Touchpoint in 2018 to continue her work with the older adult population and their families.
Kara lives on the south side of Indianapolis with her husband and her cat. She enjoys playing the guitar, singing, photography and yoga.
Lindsey Turner, MSW, LSW
Lindsey is passionate about work in human services. She obtained a bachelor’s degree in health administration and a minor in community health in 2008, followed by a master’s degree in social work. Before joining Touchpoint Senior Living, she worked in the social services field in various different capacities, from providing therapeutic services to serving as a care coordinator for patients transitioning from inpatient to outpatient settings.
In her spare time, she looks forward to spending quality time with her husband, daughter, and puppy.
Brandi Wash, FNP
Brandi Wash is a certified family nurse practitioner and is licensed by the state of Indiana to provide family-centered healthcare to patients of all ages. Brandi’s educational background includes a bachelor’s degree and a master’s degree from Indiana Wesleyan University. Brandi joined Touchpoint in April 2012. Before completing her nurse practitioner degree, she worked in the hospital and the long-term care setting.
Brandi Wash is married to her husband Levy and she has three young girls. In her free time she enjoys traveling and watching sports and spending time with her family and friends. She is excited to be a member of the Community team and loves working for Touchpoint and taking care of people in the extended care facilities.
