Joint Care Coordinators

Clinical staff at each Center for Joint Health include a dedicated joint care coordinator - a nurse certified in orthopedics and joint education who manages patient care and discharge.

Yvonne Hacker, RN

Yvonne Hacker, RN, joint care coordinator, northYvonne Hacker, RN, is the joint care coordinator at Community Hospital North’s Center for Joint Health. Hacker has over 35 years experience in providing care to orthopedic patients at Community Hospital North. Her expertise includes pre- and post-operative total joint education. She is orthopedic certified.

Yvonne received her nursing degree from the University of Indianapolis and is a member of Sigma Theta Tau National Nursing Honor Society.

Yvonne resides in Noblesville and enjoys spending quality time with her family, including two adult children.

Jennifer Harrison, MSN, RN

Jennifer Harrison, joint coordinator, HowardJennifer Harrison, MSN, RN, is the joint care coordinator for the Center for Joint Health at Community Howard Regional Health in Kokomo.

Jennifer has more than 25 years of experience working in the operating room in a variety of different roles, advancing her career from surgical tech to director of surgery. She has been in her current role since 2015, providing care to orthopedic patients across the Howard County area.

Jennifer has a bachelor’s degree in nursing and a master’s degree in nursing healthcare administration from Indiana Wesleyan University.

Jennifer is a member of the National Association of Orthopedic Nurses and the Association of periOperative Registered Nurses.

Jennifer resides in Kokomo and enjoys spending time with her family and being active in the community.

Eric Lowe, MBA-HCA, RN

Eric Lowe, RN, joint care coordinator, eastEric Lowe, MBA-HCA, RN, is the joint care coordinator at Community Hospital East’s Center for Joint Health; he also holds a dual role as Community Health Network’s Joint Replacement Manager. A graduate of Indiana University School of Nursing, Lowe earned his bachelor of science in nursing degree in 2008. He furthered his education in 2017 by receiving his master of business administration degree with a special focus on healthcare administration from Indiana Wesleyan University.

Eric was raised in Lebanon, Indiana and currently resides in Mooresville with his wife, Kelly, and four children, Kendall, Kelsie, Kaydence and Kassidy. He enjoys spending his spare time on the golf course and at his children’s extracurricular activities.

Eric’s nursing career started in the operating room where he gained vast experience as a circulating and scrub nurse for various orthopedic procedures including hip, knee and shoulder replacements. He is a member of the National Association of Orthopedic Nurses and has been a poster presenter at their national congress. Eric is very passionate about joint replacement surgery and is always looking to improve the outcomes for those who elect to take that journey.

Carrie Schroeder, RN

Carrie Schroeder, RN, joint care coordinator, southCarrie Schroeder, RN, is the joint care coordinator at Community Hospital South’s Center for Joint Health. Schroeder has a bachelor of arts degree in sociology from Franklin College and a bachelor of science degree in nursing from Marian College. She is a member of the National Association of Orthopaedic Nurses and Sigma Theta Tau International Honor Society of Nursing.

She has worked at Community Health Network since 2007. Her expertise includes pre- and post-operative total joint education. Her passion is assisting her patients in their journey to living life to the fullest without joint pain.

Carrie lives in Franklin, Indiana with her husband and two children. She is an avid runner.

Jessica Bannon, RN

Jessica Bannon, joint care coordinator, Community AndersonJessica Bannon, RN, is the joint care coordinator for the Center for Joint Health at Community Hospital Anderson. A graduate of Indiana University School of Nursing, Bannon earned her bachelor of science in nursing in 2016. Bannon also earned a bachelor of arts degree in journalism from Indiana University in 2001. Bannon has worked for Community Hospital Anderson since 2018.

Bannon was born and raised in Anderson, IN. She is active in her community volunteering for several not-for-profit board of directors including the Anderson Education Foundation, the Anderson Symphony Orchestra, CASA and more. She was named the "Best Frontline Healthcare Worker" by The Herald Bulletin in 2022. She is married to a wonderful man since 2009 and proud of her two stepsons, nieces and nephews.
