Stroke Rehabilitation

Community Rehabilitation Hospitals have comprehensive programs and services for all phases of stroke rehabilitation, with an emphasis on treatment, patient and family education, and stroke prevention. Upon admission to the hospital, all patients will complete a comprehensive evaluation process by a team of individuals who specialize in stroke care. This evaluation includes physical, occupational and speech therapy as well as physician, nursing, and if necessary, neuropsychological services.

A treatment plan, established and agreed upon by the treatment team, patient and family will be in place within seven days of admission. Patients will participate in individual and group therapy sessions on the unit, within the hospital and in the community, as appropriate to meet the individual's goals. The interdisciplinary team meets every week to update goals, adjust treatment plans and notify the payor of progress. Families are updated on an ongoing basis by the case manager.

During their stay, patients and their families have an opportunity to visit with a stroke survivor. These peer visitors provide education, wellness information, advice and support during the difficult periods of stroke recovery.

Stroke Support Group

Monthly stroke support groups are hosted at Community Rehab Hospital North and Community Rehab Hospital South. Groups are open to anyone who is recovering from a stroke and their caregiver.

When: Third Tuesday of each month from 2 to 3 p.m. and fourth Wednesday of each month from 5:30 to 6:30 p.m.
