The office is located off Shadeland Avenue between 75th Street and 82nd Street, two blocks south of Community Hospital North, on the west side of the street. Turn into the main entrance (northern entrance) of the Hillsdale Business Park (across the street from Lake Castleton Apartments) onto Hillsdale Court. Take the first driveway to the left (just after mailboxes), then park to the right (west). The sign on the building is Community Health Network (Body Zone is to the left/east end of the building).
Community Hospital East

Enter through main entrance/lobby. Go right at information desk and through double doors. Turn left into the new building. Follow signs for Emergency Room and Heritage Café, then only follow to Heritage Café. Go past the café to the elevators labeled Floor 1, Building 3. Use the elevators to your left. Go to the 3rd floor and turn left as you exit the elevator. Make another immediate left and go down the hallway, through the double doors. The office is near the end of the hallway on your left. There are chairs at the end of the hall to wait if the office door is closed.

EAP clients should utilize the CHS Emergency Department entrance. Enter through the Emergency Dept. and turn right for approximately 50 feet. The EAP office is on the right-hand side just past the hospital’s chapel. There is a waiting area with a telephone for clients to notify us of your arrival.

Use the east entrance. Once inside, suite A-125 is to the immediate left in the lobby. The first office door on the right is the EAP office. If you need help locating the EAP office, please call the therapist desk at 765-453-8615.
