Diabetes program

Managing your diabetes

Community Howard Regional Health System can help you learn how to manage your diabetes through educational programs and access to hospital and community resources and support groups. The hospital's Diabetes Program staff consists of a diabetes nurse educator, registered dietitian, certified social worker and a counselor.

Outpatient diabetes education

Sessions are held at the Diabetes Office on the Main Campus. These sessions provide the opportunity to understand your diabetes. They are available in the daytime. Family and friends are welcome to attend classes with you. In these sessions you will learn about:

  • General facts about diabetes
  • Concerns and coping skills
  • High/low blood sugar
  • Oral agents
  • Insulin, types and techniques
  • Blood glucose monitoring
  • Meal planning
  • Exercise
  • Foot care
  • Sick-day rules
  • Complications
  • Travel considerations
  • Insulin pump startups (Learn how to program your individual insulin pump.)
  • Continuous Glucose Monitoring System (72-hour diagnostic test to be interpreted by physician)

Qualifying for these sessions

A physician's order is required for these sessions. Medicare, Medicaid and most insurance companies will cover them. However, some insurance companies may require pre-certification or pre-authorization. Contact the nurse educator at 765-453-8369 for questions you may have about insurance coverage. Self-pay is also accepted, and a payment schedule may be arranged by contacting Patient Finance at 765-453-8422.

Diabetes education series

A diabetes education series is offered quarterly. Additionally, special programs for children with diabetes will be held periodically. Guest speakers will cover various topics, for example, information on the latest diabetes supplies, equipment and medications with treatment options. Dates and times for these programs will be announced via the local media, or you may call 765-453-8369 for further information.
