James D. Callahan, M.D.

James D. Callahan, M.D. Dr. Callahan is a graduate of the Indiana University School of Medicine and completed his residency in neurosurgery at IU Medical Center. Prior to joining Community Physician Network in January 2013, he was Chief of Neurosurgery at Methodist Hospital.

Dr. Callahan is a board-certified neurosurgeon with 17 years’ experience including two years on active duty as a neurosurgeon in the Air Force. He has special interest in cranial cases, complex spine, minimally invasive spine procedures and spine trauma. He provides neurosurgical care to patients at Community Hospital Anderson.

Dr. Callahan is married, has three sons and is a native of West Lafayette, Indiana.


John T. Cummings, Jr., M.D.

John T. Cummings, Jr., M.D.Dr. Cummings joined Community Neurosurgery in 2010. He is a graduate of the Indiana University School of Medicine and is a board-certified neurosurgeon. He has provided complete neurosurgical care to patients at Community Health Network for more than 20 years. In 2010, Dr. Cummings was named the medical director of neurosurgery for Community Health Network. As medical director, Dr. Cummings leads the neurosurgical team in the ongoing growth and development of comprehensive neurosurgery services at Community Hospitals East, North, South and Anderson.

He is a leading innovator for minimally invasive treatment for spine and brain disorders. Dr. Cummings also treats normal pressure hydrocephalus (NPH) and nerve entrapment syndromes such as carpal tunnel syndrome.

Dr. Cummings is married and has three sons.


Jill W. Donaldson, M.D.

Jill Donaldson, M.D.Dr. Donaldson comes to Community Health Network from the Arnett Clinic in Lafayette, Indiana, where she practiced for more than five years. She earned her doctorate in medicine (M.D.) and completed her neurological surgery residency at Indiana University School of Medicine. Originally from Winter Park, Florida, Dr. Donaldson received a bachelor degree in chemistry from Hollins College in Roanoke, Virginia.

She is a member of many professional organizations including the American Association of Neurological Surgeons, the Congress of Neurological Surgeons, Alpha Omega Alpha, Phi Beta Kappa, and the American Medical Association. Her specialty areas of interest include surgical management of complex spinal disorders, neoplasms of the brain and spine, trigeminal neuralgia, hydrocephalus and peripheral nerve entrapment.

In her time off, Dr. Donaldson enjoys spending time with her husband, playing piano and bass, and competing in equestrian show jumping events.

Dr. Donaldson was a featured neurosurgeon in the 2010 Indianapolis Monthly "Top Doctors" listing of the area's leading physicians. 

David C. Hall, M.D.

David Hall, M.D.Dr. David C. Hall joined Community Neurosurgery on February 1, 2010. Dr. Hall is a graduate of Westminster Medical School, University of London, London, England and completed his surgical internship and neurosurgical residency at IU Medical Center. He is board certified and brings over 25 years of experience from the Indianapolis area. In addition to his surgical practice, Dr. Hall served as Clinical Assistant Professor of Neurosurgery at IU Medical Center.

Dr. Hall’s interests include cranial and spinal oncology, movement disorder surgery for Parkinson's Disease and tremors, as well as minimally-invasive spine techniques for trauma and degenerative disease.

Dr. Hall recently was named to the 2011 Indianapolis Monthly "Top Doctors" listing of the area's leading physicians for neurological surgery and named in the 2013 and 2012 Best Doctors in America® for neurological brain surgery in Indiana.

Daniel L. Kim, M.D.

Daniel L. Kim, M.D.Dr. Kim joined Community Physician Network, Neurosurgical Care in July 2013. He is a graduate of the Indiana University School of Medicine, and he also completed his neurosurgical residency at the Indiana University School of Medicine. During his training, Dr. Kim finished an enfolded fellowship in complex spinal disorders.

Dr. Kim is a general neurosurgeon and has special interests in spine disease, brain tumors (including skull base lesions), trigeminal/glossopharyngeal neuralgia and hemifacial spasm, peripheral nerve problems, endarterectomy, and vascular disease. Dr. Kim is board-eligible and provides neurosurgical care for patients at Community Hospital Anderson.

He is married and enjoys fishing, biking and playing golf in his free time. Prior to medical school, Dr. Kim was an attorney and practiced municipal finance law with Ice Miller Donadio & Ryan. He has served on various local community boards including the Neighborhood Christian Legal Clinic and the Shalom Health Clinic, and he believes strongly in community involvement.

Dr. Kim is happy to be a part of Community Physician Network and is excited to provide top-notch specialty care for patients.


Matthew A. Rendel, M.D.

Matthew A. Rendel, M.D. Matthew Rendel, M.D., completed his medical degree at Indiana University School of Medicine. He did his neurosurgical residency at University of Kansas Medical Center in Kansas City and then served on the faculty. He is a general neurosurgeon with special interests in complex spinal disorders, brain tumors and vascular disorders. Dr. Rendel is a board-eligible neurosurgeon who will provide care at Community Hospital South and Johnson Memorial Hospital in Franklin, Indiana.

Dr. Rendel is originally from Southern Indiana. He is a fourth-generation Indiana physician and recently moved home to Indiana with his wife.

Robert B. Sloan, Jr., M.D.

Robert Sloan, Jr., M.D.Dr. Sloan is a graduate of the Indiana University School of Medicine and completed his residency in neurological surgery at Indiana University. He provides complete neurosurgical care for patients of Community Health Network. His interests include minimally invasive treatment for degenerative spine disorders, treatment of primary and metastatic brain tumors, pituitary tumors, chiari malformation, vagal nerve stimulation, normal pressure hydrocephalus and peripheral nerve disorders.

He and his wife Jennifer, also a physician, have three children, and are lifelong residents of Central Indiana.
