Bridges to Health

Bridges to Health is an education and monitoring program designed to help you or your loved ones better manage health issues such as asthma, diabetes, etc. The Bridges to Health program is being offered to you, on a voluntary basis, as a way to work with various members of your health care team to control your health issues and thereby increase your overall wellness—both now and in the future.

Program benefitsWellspring Pharmacy

If you participate in the Bridges to Health program, one immediate benefit is that you (or your dependants) will be eligible to receive your prescriptions for free through a copay waiver. Copay waivers will go into effect after the completing education and monitoring steps based upon your health issues.

You will also receive regularly scheduled consultations and be provided with the following:

  • Individualized counseling sessions, designed to help you better control your condition
  • Help in making small, gradual changes in your daily routine that can improve your health
  • Important instruction on the use of your medications

Asthma management

The Bridges to Health asthma education and monitoring program is designed to help you or your loved ones better manage asthma. Historically, it was thought that asthma was an intermittent illness that was characterized by episodes of wheezing, shortness of breath, chest tightness or coughing. It is now known that asthma is a chronic disease and that airway inflammation is an early and persistent feature of the illness. If left unchecked, this inflammation can lead to permanent structural damage in the lungs, resulting in irreversible breathing difficulties.

Cholesterol management

The Bridges to Health cholesterol education and monitoring program is designed to help you or your loved ones better manage cholesterol. In the United States, approximately one person every minute will die from a heart attack. Studies have shown that elevated levels of low-density lipoprotein (LDL) cholesterol is a major risk factor for heart disease and it can be lowered with diet, exercise and medications. It is estimated that up to 90 percent of patients are not being treated optimally, and therefore are at risk for a heart attack.

Diabetes management

The Bridges to Health diabetes education and monitoring program is designed to help you or your loved ones better manage diabetes. Studies have shown that patients who tightly control their diabetes early in its progression have much better outcomes than patients who don’t achieve good control of their disease.
