Palliative Care at Home

Palliative Care at Home is a palliative care program that specializes in symptom management. We work closely with your care team to help you manage your serious illness at home and support you in the following areas:

  • Coordinating care among your entire healthcare team
  • Managing pain and symptoms such as fatigue, shortness of breath and loss of appetite
  • Communicating with physicians and pharmacists
  • Dealing with side effects from treatment or medications

Who Can Benefit?

Palliative Care at Home benefits people who are primarily restricted to their home because of pain and symptoms related to serious illness. It differs from hospice or senior care programs, because it supports people managing a serious illness no matter their diagnosis or age.

If you received palliative care during your stay in a Community Health Network hospital, Palliative Care at Home will ensure an easy transition to care at home. Talk to your doctor about a referral.
