Dr. Thomas Ledyard named Life's Journey: Choices in Care medical director

INDIANAPOLIS – Community Health Network recently named Thomas Ledyard, M.D., as medical director of Life’s Journey: Choices in Care, Symptom Management Group and Community Home Care.

In his new role, Ledyard will focus on connecting and coordinating palliative medicine services, from inpatient to outpatient to home care to hospice. He will also continue to be at the bedside for Life’s Journey: Choices in Care, continuing his work in the hospitals.

Ledyard has been with Life’s Journey, Choices in Care, since 2010, previously serving as associate medical director.

Prior to joining Life’s Journey: Choices in Care, Ledyard served as medical director for Jane Pauley Community Health Center and community benefit school based clinics. He also served as a faculty member in Community Health Network’s Family Medicine Residency Program.

Ledyard completed medical school at Michigan State University. He also completed a hospice and palliative care fellowship through self-study and active preceptorship, passing the boards in 2012.

His affiliations and community involvement include the American Academy of Family Physicians, American Society of Addiction Medicine, National Acupuncture Detoxification Associate, and Coalition for Homeless Intervention and Prevention.
