Community Physician Network welcomes new providers

Indianapolis, IN--- Community Physician Network, the integrated, multi-specialty physician group at Community Health Network, welcomes the following providers:

Samantha Bouchie, M.D., is an internal medicine physician who has special interests in preventive health care, women’s health and infectious diseases. She completed her medical degree at Indiana University School of Medicine and her internal medicine residency at St. Vincent Hospital, where she was chief resident. Dr. Bouchie’s office is located at 9669 E. 146th Street, Suite 200, Noblesville.

Rehan Haque, M.D., is a board-certified pulmonary and critical care physician who has special expertise in endobronchial ultrasounds. He completed his medical degree at Aga Khan University School of Medicine in Pakistan and his internship and residency in internal medicine at Tufts University in Boston, Massachusetts. He then completed a pulmonary and critical care medicine fellowship at Penn State University Hershey Medical Center in Hershey, Pennsylvania. Before joining Community Physician Network, Dr. Haque worked as affiliate assistant professor of medicine at the University of Miami Miller School of Medicine at the West Palm Beach VA Medical Center, where he was also chief of the pulmonary and critical care section. Dr. Haque’s office is located at 1400 N. Ritter Avenue, Suite 375, Indianapolis.

Rachel Morton, M.D., is a pediatric hospitalist who has special interests in the management of asthma exacerbations, wound closures, and reductions of fractures and dislocations. She completed her medical degree at New York Medical College and her residency at Maria Fareri Children’s Hospital, both in Valhalla, New York. Prior to joining Community, Dr. Morton specialized in pediatric urgent care at a practice in New York. Dr. Morton cares for patients at Community Hospital North, 7150 Clearvista Parkway, Indianapolis.

Lauren Rau, M.D., is a pediatric hospitalist with procedural interests in conscious sedation and lumbar punctures. She completed her medical degree at Indiana University School of Medicine and her pediatric residency at Riley Hospital for Children. Dr. Rau treats patients in the special care nursery at Community Hospital North, 7150 Clearvista Parkway, Indianapolis.

Carla Weaver, M.D., is a pediatric hospitalist who was born and raised in Goshen, Indiana. She completed her medical degree at IU School of Medicine and her pediatric residency at Methodist Children’s Hospital in Indianapolis. She completed a two-year fellowship in primary care sports medicine at Methodist, during which time she traveled to Brazil and Cancun with sports team and also served as the team physician for the NCAA runner-up Purdue women’s basketball team in 2001. After working in student health at Purdue for two years, she worked St. Vincent as a newborn hospitalist for 10 years, where she was involved in establishing the Neonatal Rapid Response Team protocol and was a member of the Neonatal Abstinence Syndrome Task Force. Dr. Weaver has a special interest in caring for late preterm infants and is pleased to join Community Physician Network, serving patients in the special care nursery at Community Hospital East. 1400 N.
