Community Physician Network welcomes new providers

Indianapolis, IN--- Community Physician Network, the integrated, multi-specialty physician group at Community Health Network, welcomes the following providers:

Jason Everman, DO, is a family medicine physician who concentrates on preventive health care and treating the body as an integrated whole. He completed his medical training at Midwestern University, Chicago College of Osteopathic Medicine, in Downers Grove, IL, and his family medicine residency at St. Vincent Hospital in Indianapolis. Dr. Everman’s office is located at 7910 E. Washington Street, Suite 200, Indianapolis.

Timothy Kelly, MD, is a board-certified internal medicine and addiction medicine physician. He has been a consulting physician for Community for more than 20 years and comes to Community Physician Network after serving as director of medical services at Fairbanks Hospital for 35 years. He is a graduate of the Indiana University School of Medicine. Dr. Kelly sees patients at Community’s Behavioral Health Pavilion, 7165 Clearvista Parkway, Indianapolis.

Isra’a Khan, MD, is a pediatric hospitalist at Community Hospital North. She received her medical degree at Dow Medical College in Karachi, Pakistan, and her pediatric residency at Hurley Children’s Hospital, Michigan State University, in Fling, MI.

Michael Koelsch, MD, is a board-certified thoracic and cardiovascular surgeon who received his medical degree at Finch University of Health Sciences, The Chicago Medical School. He completed his general surgery residency and fellowship in thoracic and cardiovascular surgery at Loyola University, Foster McGraw Hospital, in Maywood, Illinois. Dr. Koelsch’s office is located at 8075 N. Shadeland Avenue, Indianapolis.

Allison Sturtevant, MD, is a hospitalist who was born and raised in Fishers, IN. She first joined the Community team as a cardiac catheterization lab technician after graduating with a bachelor’s degree in biology from the University of Indianapolis. While working at Community, she earned a master’s degree in physiology at IUPUI and a doctorate of medicine from the Indiana University School of Medicine. Subsequently, she completed her internal medicine residency at Tulane Medical Center in New Orleans, LA, then accepted a position as a traveling physician in Louisiana, Iowa and Ohio. She returns to Community Physician Network as a hospitalist covering Community Hospital North and Community Rehabilitation Hospital.
