Community Physician Network welcomes new providers

Indianapolis, IN---Community Physician Network, the integrated, multi-specialty physician group at Community Health Network, welcomes the following providers:

Elizabeth Beach, M.D., is a pediatrician who is returning to practice at Community after staying home with her children for the past three years. Dr. Beach completed her medical degree at the University of Cincinnati School of Medicine and a residency in pediatrics at Riley Hospital for Children. Her office is located at 9669 E. 146th Street, Suite 250, Noblesville.

David K. Booth, D.O., is a board-certified family medicine physician who enjoys seeing patients of all ages and has a special interest in preventive care. Dr. Booth completed his medical degree at Lake Erie College of Osteopathic Hospital in Harrisburg, Pennsylvania, and a residency in family medicine at Community General Osteopathic Hospital, Pinnacle Health, in Harrisburg, Pennsylvania. He comes to Community after working in private practice in Meadville, Pennsylvania. Dr. Booth’s office is located at 8920 Southpointe Drive, Suite B, Indianapolis.

Michael DaRosa, D.O., is a primary care sports medicine physician as well as the team physician for Franklin Central and Beech Grove High Schools. He specializes in non-operative treatments of musculoskeletal conditions including acute, chronic and overuse injuries such as sprains, strains, tendonitis and fractures. Dr. DaRosa completed his medical degree at Des Moines College of Osteopathic Medicine and his residency at Exempla St. Joseph Family Medicine in Denver, Colorado. He completed fellowship training at Indiana University Primary Care Sports Medicine Fellowship Program. Dr. DaRosa has two convenient office locations: 10122 E. 10th Street, Suite 100, Indianapolis, and 555 E. County Line Road, Suite 107, Greenwood.

Vin Gupta, M.D., is a pediatric hospitalist at Community Hospital North. Dr. Gupta completed his medical degree and pediatric residency training at the Medical College of Ohio, and a fellowship in pediatric critical care medicine at Duke University Medical Center/Duke Children’s Hospital. His clinical interests include conventional and non-conventional mechanical ventilation and procedural sedation. He is board-certified in both pediatrics and pediatric critical care medicine.

Syeda Naqvi, M.D., is a geriatrician who works alongside the primary care provider to address seniors’ complex medical, physical and psychosocial needs. Dr. Naqvi completed her medical degree at Sind Medical College in Karachi, Pakistan, and her residency at the University of Kentucky Family Medicine Program in Hazard, Kentucky. She also completed a fellowship in geriatrics at the University of Louisville. Dr. Naqvi’s office is located at 1400 N. Ritter Avenue, Suite 231, Indianapolis.

Nicole Zulkowski, M.D., is a physical medicine and rehabilitation physician who cares for patients at Community Spine Center. Dr. Zulkowski completed her medical degree and residency at Indiana University School of Medicine. She is a member of the American Academy of Physical Medicine and Rehabilitation, American Medical Association, Indianapolis Medical Society and Indiana State Medical Association. Dr. Zulkowski’s office is located at 555 E. County Line Road, Suite 107, Greenwood.
