Medicare Part D Help Sessions

Details of sessions for 2025 plan help will be posted in fall 2024.

Community Health Network is providing FREE Medicare Part D help sessions to see if you can lower your medication costs.

This year, both in-person and virtual appointments (either by phone or computer) are available. To reserve your appointment time, call 317-355-6913 and indicate your preference.

Medicare Part D plans offer prescription drug coverage to everyone with Medicare and may help decrease the cost of your medications. The cost of these plans change every year, so come and discover which 2024 Medicare Part D plan may be best for you and your budget. Trained pharmacists, medication assistance coordinators, and SHIP counselors will be here to answer your questions and give you information based on your specific needs.

Please have the following information with you to reference during your Medicare help session:

• Your Medicare “red, white, and blue” cards
• Your Medicare Part D card, if you have one
• Your Medicare.gov username and password, if you have one
• Your medications
• Your health insurance cards
• Any letters or materials that you received in the mail from Medicare or Social Security
