Continuum of Care
Our continuum of care has three divisions, providing care from chronic to acute conditions for ages five years to seniors suffering from mental illness and or substance abuse.
Clients may be seen in a variety of locations, which include inpatient, residential, outpatient, school, community and home-based settings.

Inpatient care provides a safe hospital setting for assessing and stabilizing patients experiencing acute psychiatric difficulties and substance abuse problems. Services include treatment programs designed for adults. The clinical team of psychiatrists, nurses, social workers and occupational therapists assists patients onto the path of recovery with effective discharge planning. They determine the appropriate aftercare programs and ensure continuity of treatment for patients.
Daily visiting hours: 12:30 to 1:30 p.m. and 7 to 8 p.m.
- We encourage visitation; however, we ask that the visiting hours be respected.
- Children under the age of 14 years are not permitted to visit the units, unless special arrangements are made.
- Confidentiality is expected.
- Any visit which becomes non-therapeutic may be terminated.

Outpatient clinic hours are geared to meet patient clinical needs. As such, various clinics maintain hours between 8 a.m. to 9 p.m. Monday through Thursday, and 8 a.m. to 5 p.m. on Fridays. Depending on demand, clinics may offer some limited Saturday hours.

Community Howard conducts community outreach and client service engagement for homeless individuals and families on the streets, in service venues, and other locations where they can be found in Howard County, Indiana.
Through the PATH Grant, the outreach team responds to the issues and concerns of people in need of assistance as well as local residents, businesses, and others relative to homelessness in this community. Focused on the "hardest-to-reach" chronically homeless population, the outreach team works in partnership with city staff, local service providers, health professionals, law enforcement, and the community at-large to help people overcome chronic homelessness and connect with the services they need to gain stable housing and achieve self-sufficiency.
