Meet our staff

Brian S. Foley, M.D., MBA
Medical Director

Brian S. Foley, M.D., MBADr. Foley, medical director for the Community Spine Center at Community Health Network, has extensive experience with Indiana worker’s compensation and routinely performs Independent Medical Exams and impairment ratings. In addition to treating injured workers, he has lectured internationally on a wide range of topics. He has performed thousands of diagnostic and therapeutic spinal injections under fluoroscopy.

Dr. Foley is a member of the International Spine Intervention Society. He is board-certified in physical medicine and rehabilitation, electrodiagnostic medicine and pain medicine. Along with Dr. Sorg, he is one of only two physicians in Indianapolis to have been recognized by the NCQA's Back Pain Recognition Program.

Dr. Foley received his bachelor of arts in chemistry from Binghamton University; he earned his medical degree from the State University of New York at Syracuse. Dr. Foley performed his internship in internal medicine and residency in physical medicine and rehabilitation at Ohio State University. He practiced outpatient musculoskeletal medicine and interventional spine work in private practice before serving as an assistant professor at Indiana University School of Medicine. Dr. Foley earned his MBA from the Indiana University Kelley School of Business.

Nathan Kurtz, M.D.

Nathan Kurtz, M.D.Dr. Nathan Kurtz, a physical medicine and rehabilitation physician, joined Community Spine Center in 2015 and serves patients in the north office. He is a graduate of Indiana University School of Medicine, where he also completed his residency in Indianapolis. Dr. Kurtz received his undergraduate degree from Goshen College, Goshen Indiana.

Dr. Kurtz is a member of the American Academy of Physical Medicine and Rehabilitation and American Medical Association.

During his free time he enjoys gardening, pottery, drawing, reading, watching movies, and spending time with family and friends.

Joshua Salyer, D.O.

Joshua Salyer, D.O.Dr. Salyer joined Community Spine Center in 2011 and serves patients on the south side of Indianapolis. He is a graduate of Midwestern University-Arizona College of Osteopathic Medicine in Glendale, and he completed his residency in physical medicine and rehabilitation at Indiana University School of Medicine. Dr. Salyer is a member of the American Academy of Physical Medicine and Rehabilitation, American Medical Association, American Osteopathic Association and International Spine Intervention Society.


Jason Sorg, M.D.

Jason Sorg, M.D.Dr. Sorg joined the Community Spine Center at Community Hospital North in 2008 after completing his residency in physical medicine and rehabilitation at the Indiana University School of Medicine. He received his bachelor of science degree in chemistry and master of science degree in physical therapy from the University of Indianapolis prior to attending medical school at the Indiana University School of Medicine. During medical school, Dr. Sorg was inducted into the prestigious medical honor society, Alpha Omega Alpha.

In addition to specializing in providing care to patients with neck and back pain, Dr. Sorg’s areas of interest include electromyography, fluoroscopic guided spine injections, occupational health and sports medicine.

Dr. Sorg is a member of the American Association of Physical Medicine and Rehabilitation and International Spine Intervention Society. Along with Dr. Foley, he is one of only two physicians in Indianapolis to have been recognized by the NCQA's Back Pain Recognition Program.

Dr. Sorg lives in Carmel, Indiana. In his free time, he enjoys spending time with his family, reading, attending sporting events and running.

Nicole R. Zulkowski, M.D.

Nicole R. Zulkowski, M.D.Nicole Zulkowski joined Community Spine Center in 2012 and serves patients on the south side of Indianapolis in the Greenwood office. She received her undergraduate degree from University of Indianapolis and her medical degree from Indiana University School of Medicine. Dr. Zulkowski also completed her residency at Indiana University School of Medicine in Indianapolis. She is board-eligible in physical medicine and rehabilitation.

Dr. Zulkowski is a member of several professional organizations, including the American Academy of Physical Medicine and Rehabilitation, American Medical Association, Indianapolis Medical Society and the Indiana State Medical Association.

Nurse practitioner

At Community Spine Center, our nurse practitioner (NP) partners with our physicians to review the patient history and conduct physical exams. The NP may then order diagnostic tests and treatment to develop an individualized medical plan. The NP can prescribe medications and make referrals as needed for acute and chronic medical conditions.

Lauren Williams, NP

Lauren Williams, NPLauren Williams, NP, has joined Community Spine Center to provide care for patients at the North location. Lauren completed her nurse practitioner training and master’s degree in 2013 from Ball State University, Muncie. She received a bachelor degree in psychology from University of Kentucky and has a bachelor’s degree in accelerated nursing from Indiana University Purdue University, Indianapolis (2007).

Bringing a wide range of patient care experience, Lauren has expertise in working with patients and their families to educate about treatment plans, goals and prevention, as well as health and wellness topics.

Lauren is a member of the American Association of Nurse Practitioners and Coalition of Advanced Practice Nurses of Indiana.

In her free time, Lauren enjoys spending time with her family, exercising and cooking, and is involved in her church. She and her husband have a son and daughter, and reside in Fishers.

Physical therapists

All our physical therapists are certified in the McKenzie Method and promote the highest standards of using the McKenzie Method of Mechanical Diagnosis and Therapy MDT protocols.
