Important Update About Primary Care Services

Our commitment to providing exceptional care is unwavering. Although the way we deliver care is changing in response to COVID-19, our dedication to the health of you and your family remains the same. As counties in Indiana enter stages of reopening, so does our health network.

The integration of MedCheck and Primary Care services allowed us to extend hours and provide access to care when you need it most, while maintaining safety precautions for all. At this time we are slowly reopening Primary Care and MedCheck locations to best serve the community.

We have implemented many precautions to protect you from infection while under our care. These safety measures include:

  • Health screenings of each patient and visitor upon arrival
  • Thorough and frequent disinfection of surfaces and materials
  • Rigorous and frequent handwashing
  • Separation of potential COVID-19 patients and those with non-respiratory symptoms
  • Ability for individuals to social distance
  • Continued visitor restrictions
  • Extensive PPE worn by all caregivers

We’re here for you; You can always call your primary care provider if you’re looking for an appointment. They will determine whether you need to be seen in person or can have a telehealth appointment.

For appointments being rescheduled, please expect a phone call from your provider. Patients will be prioritized based on when the original appointment was booked and the urgency in which it holds.

If you do not have an established Community primary care provider, you may call 317-621-2727 to be directed to care. Our team of clinical professionals is here to listen carefully to your individual needs and direct you to the best care, whether that’s a phone visit with a Community provider, a virtual visit, or an in-person visit at a Community location.
