Our commitment to providing you exceptional care is unwavering, and though the way we deliver care may change temporarily during our response to COVID-19, our dedication to the health of you and your family is unchanged.

Integrated Care to Better Serve You

We integrated our MedCheck and Primary Care services to extend hours and provide access to care when you need it most. Our care sites are now open from 9 a.m. to 9 p.m. Please note online appointments are temporarily unavailable.

For any care need you may have, please call 317-621-5500.

If you have an established primary care doctor with Community, you can still call your regular office number to receive care.

Our team of clinical professionals is here to listen carefully to your individual needs and direct you to the best care, whether that’s a phone visit with a Community provider, a virtual visit, or an in-person visit at a Community location.

Virtual Primary Care

In order to meet your primary care needs, we are offering telephone and video visits.

We ask that you do not visit a primary care office in person unless specifically instructed to by your provider. By reducing the number of people in our waiting rooms and facilities, we’re protecting you against potential exposure to COVID-19.

If you have a scheduled primary care appointment:

Your primary care doctor will reach out to you about a phone or virtual appointment. Please call your doctor’s office with questions about your appointment.
