Care at Community - More Ways to Get Exceptional Care

Exceptional care goes beyond our walls at Community. It’s our promise to you, no matter where, when or how you need medical attention. With convenient ways to access care in person and online, we’re always ready to stand by your healthcare needs with your safety in mind.

We’ve made several changes at each of our locations to ensure the safety and well-being of our patients and staff. This includes enhanced cleaning and sanitizing, social distancing and infection prevention protocols that align with CDC recommendations.

Here are some of the ways you can access the exceptional care you expect from Community Health Network.

Getting Care from Your Provider

Your primary care provider is your partner in health, and that hasn’t changed during the pandemic. If you already have a Community provider, they’re your go-to for answers on your continued care.

If you have questions or concerns about your health — like getting scheduled for immunizations or your annual checkup — you have options. Call your provider’s office or log in to MyChart to schedule virtual or in-person appointments. With MyChart, you can also check your health records, refill prescriptions or message your care team right from this device.

Urgent Care

There are some times when care can’t wait. MedChecks and Community Clinic at Walgreens locations offer convenient care close to home, and they’re now taking appointments.

For the safety of all patients and visitors, walk-in services are not available. Instead, you can schedule your visit in advance using Save a Spot.

Visit this page to Save a Spot at MedCheck. Please note that Fishers and Carmel locations aren’t currently seeing patients. Call 317-621-2727 to connect to care in those areas.

Health Risk Assessments

It’s always a good idea to keep an eye on your overall health, even if you aren’t feeling any symptoms. Our Health Risk Assessments help you determine your risk for heart disease, cancer and other serious conditions.

Taking an assessment is quick, and you’ll get personalized feedback with actionable steps for your health. It’s also a great conversation starter for your next visit with your provider.
