If you are interested in applying for a Community Health Network residency program, please use PhORCAS to submit all application materials. These will include a letter of intent, curriculum vitae, three references (please use ASHP form), and official college transcript(s).

Application materials should be received no later than January 7, 2019 at 11:59 p.m. Upon evaluation of application materials, qualified applicants will be offered an on-site visit and interview with the residency advisory team. Interviews take place in late January and February. Successful candidates will be expected to have a Doctor of Pharmacy degree, have an active BLS certification and obtain Indiana licensure by August 1 of the residency year. PGY2 candidates are expected to have completed an ASHP-accredited PGY1 residency. The application process will include a formal employment application to be completed upon successful matching with our program.
