Christine Jung, DO
Dr. Jung is originally from southern California. She completed her undergraduate education at University of California, Irvine with a bachelor's in information and computer science. Dr. Jung worked as a computer programmer for about two years before her husband, Dr. Peter Choi, convinced her to pursue medicine. She attended Touro University of Nevada College of Osteopathic Medicine before joining the Community Westview Family Medicine program. Dr. Jung enjoys watching movies with her husband, playing with her two dogs and exploring all that Indiana has to offer.
Jeffrey Klak, DO
My goal is to provide comprehensive and compassionate medical care. I believe in patient education, preventive care, and in-depth education and treatment. I strive to actively involve my patients in their medical care and decision-making process to promote improved quality of life, health and wellness. It is my passion to provide comprehensive, goal-oriented primary care.
I obtained my undergraduate degree on full scholarship as an Evans Scholar at Indiana University in Bloomington with a major in biology. I went on to graduate from DeBusk College of Osteopathic Medicine in Harrogate, Tennessee. My interests include golf, roller-blading, fishing, grilling and spending time with my three children and my lovely wife.
Steven Patton, DO
Dr. Patton was born in Chicago, Illinois and raised in Covert, Michigan. He graduated from Kentucky State University with a bachelor’s of science in biology. He then attended the University of Pikeville Kentucky College of Osteopathic Medicine and earned a doctorate of osteopathic medicine. He enjoys volunteering, spending time with his daughter, practicing martial arts and listening to music.

I am originally from Avon, Indiana on the west side of Indianapolis. I attended undergrad at Indiana Wesleyan University in Marion were I studied biology. I graduated medical school from the West Virginia School of Osteopathic Medicine in Lewisburg, West Virginia. My wife and I moved back to the Indianapolis area and I am a currently a resident physician at Community Westview Hospital. I’m excited to be serving as an osteopathic family medicine physician in the community I grew up in. In my free time, I love spending time with my wife on our many adventures, being outside hiking, hunting and fishing with our dog, and spending time with friends and family.
Brett Erickson, DO
Dr. Erickson is originally from Western Michigan and attended medical school at LECOM, in Erie, Pennsylvania. He is excited to be back in the Midwest to practice medicine. In his spare time, he enjoys electronics, working on cars, watching movies, as well as various outdoor activities, including walks, playing golf, and playing tennis.
Ban Gans, DO
I am a Hoosier through and through. I graduated with my bachelor and master's degrees from Ball State University in Muncie, Indiana. I then spent my only years living outside of Indiana at medical school at Midwestern University in Glendale, Arizona. My wife and I are proud to call Indianapolis our home and enjoy playing hosts to our out-of-town friends and family. As a resident at Community Health Network, I look forward to providing excellent comprehensive care to my patients and their families. When free time allows, I enjoy long-distance running (looking forward to my first marathon), weight training, camping and drumming.
Cody Walker, DO
I was born and raised in Texas where I did my undergraduate and master’s degrees at Texas Tech University. I then attended medical school at Arizona College of Osteopathic Medicine in Phoenix. I grew up playing sports and have a great interest in sports medicine, exercise and nutrition, and am an avid cyclist in my free time. I have really enjoyed Indianapolis so far because it is a large metropolitan city that has a small town feel to it. I am still discovering the good roads to bike on and great places to visit in Carmel, Broad Ripple and downtown Indianapolis. I chose Community Westview’s Family Medicine Residency Program for multiple reasons, including the wonderful people that are directly and indirectly associated with the program, the clinic at Speedway Family Physicians, and the great physician support at Community Health Network.
Peter Choi, DO
Dr. Peter Choi grew up in Diamond Bar, CA. He majored in neurobiology at University of California, Berkeley and obtained a master's degree in physiology and biophysics at Georgetown University in Washington, D.C. He graduated from Western University of Health Sciences College of Osteopathic Medicine in Pomona, CA. Dr. Choi finished PGY-1 year at University of Nevada Family Medicine Residency Program in Las Vegas before joining the Community Westview Family Medicine Program as PGY-2. He served as a chief resident in PGY-3 year and was awarded Resident Teacher of the Year.
Dr. Choi joined as an Assistant Director at Community Westview Osteopathic Family Medicine Residency Program in July 2015 and also works at Urgent Care in Lafayette, IN. Dr. Choi prioritizes patient education with emphasis on the prevention of disease and promotion of health. In his free time, Dr. Choi loves to golf, bike, cook, watch movies, and explore outdoors with his wife and two dogs.
Valerie Melton, DO

Dr. Melton joined our faculty team after graduating from the Community Westview Osteopathic Family Medicine residency. She is originally from Central Kentucky and went to Georgetown College where she received a bachelor of science in biology. She then attended the University of Pikeville Kentucky College Of Osteopathic Medicine where she earned her doctorate degree and completed an undergraduate fellowship in osteopathic manipulative medicine.
She is passionate about the value of osteopathic principles as they relate to a holistic approach to patient care and preventive medicine. Her goal is to not only treat patients with compassion and respect, but to also empower them to make positive changes for their future health. When not working, Dr. Melton enjoys cooking, gardening and biking. She treasures her family dearly and takes advantage of every opportunity to spend quality time with them. She and her husband enjoy exploring many of the activities in Indianapolis, as well as playing with their puppy, Josie.
Martha Neilsen, DO
Martha Nielsen is originally from Upper Black Eddy, Pennsylvania. She received a bachelor’s degree in biology from Ursinus College in Collegeville, PA before attending Lincoln Memorial University-DeBusk College of Osteopathic Medicine in Harrogate, Tennessee. While she loved living in both Pennsylvania and Tennessee, she wanted to live somewhere different so when she and her husband found Community Westview Hospital they knew it was for them! They live in Brownsburg with a small menagerie of pets. In their free time they enjoy watching all kinds of movies, doing projects around the house and playing with their dogs.
Joseph "JT" Silvers, DO
JT is from the small town of Monticello, Kentucky and graduated summa cum laude with a bachelor’s of science degree in biology from Campbellsville University in central Kentucky. He then went to medical school at the University of Pikeville Kentucky College of Osteopathic Medicine, where he also completed a teaching fellowship in osteopathic manipulative medicine. He enjoys fishing, being outdoors, playing music and spending time with his wife, Dana.
Kirk Hanson, DO
Kirk Hanson is a caring, dedicated, humanistic physician who puts the health, interests and well-being of his patients first. He graduated from the Kirksville College of Osteopathic Medicine in 2011. Kirk is community oriented and technologically adept. He has served as a member of the AOA Council of Interns and Residents, representing ;all postdoctoral trainees on the Information Technology Advisory Board. Kirk enjoys building relationships with patients, life coaching and problem-solving.
Prior to his healthcare education, Kirk spent 16 years working in technology and business. He has a master's degree in business administration and a bachelor of science degree in computer science. Kirk and his spouse, Angie, live in Brownsburg along with their many dogs. In his spare time, Kirk's interests include organic foods, exercise and fitness, skiing, and volunteer activities.
Dawn Meacham, DO
Dawn grew up on a small farm outside of the Southeastern Indiana town of Madison. She received a bachelor of arts degree in psychology from Butler University in 2005, and went on to study for a master's degree in public health at the University of Kentucky until beginning medical school in 2007. She graduated from medical school at University of Pikeville Kentucky College of Osteopathic Medicine in 2011 before joining the residency. In her free time, she enjoys spending time with her husband and son. They love sports, the outdoors, music, museums and traveling.
Meredith Potrzebowski, DO

Dr. Potrzebowski grew up in Marion, Indiana and graduated from Purdue University in 2007 with a degree in health sciences. She completed medical school at Lincoln Memorial University-DeBusk College of Osteopathic Medicine in Harrogate, Tennessee. During residency, she completed additional training in cranial manipulation and began to learn acupuncture. She and her husband, Paul, live in Indianapolis with their dogs Harvey and Gwyn. In her spare time, she enjoys running and being with family and friends.
Sarah Lynley Rader, DO
Dr. Sarah Lynley Rader is a native of Central Indiana. She obtained a B.A. in Political Science from Indiana University, and earned her medical degree from Kansas City University of Medicine and Biosciences. After completing her internship at the University of Colorado Medical Center in Denver, Dr. Rader returned to Indiana to continue her training through the Community Westview Family Medicine Residency. In addition to the full breadth of traditional family medicine, Dr. Rader has special interests in acupuncture and office-based dermatology, including cancer screening and cosmetic procedures. Dr. Rader's goal is to help her patients attain peak health and wellness by using all the tools now available. These include motivating and educating patients on healthy diet, exercise, disease prevention and stress management. Dr. Rader uses the same approach to facilitate optimal health and symptom control in patients with chronic disease.

Dr. Burch is a family medicine physician working as a member of Community Physician Network. He was born and raised in Loogootee, a town in southwest Indiana. After graduating from DePauw University in Greencastle, Indiana, he attended medical school at the Lake Erie College of Osteopathic Medicine in Erie, Pennsylvania. Dr. Burch felt it was important to return home to promote health and help fellow Hoosiers. Following medical school he returned to Indiana for residency at Community Westview Hospital and now works at one of Community's family medicine practices.
Jared Snyder, DO
Current practice: Wound Care
Dr. Snyder was born and raised in Indianapolis. His first experience with Community Westview Hospital was as a volunteer while attending college. After graduating from Purdue University in West Lafayette, Indiana, with a bachelor’s degree in neurobiology and physiology, he attended medical school at Lake Erie College of Osteopathic Medicine in Erie, Pennsylvania. Following medical school, it was important to him to return to the city where he was raised to continue his medical education. He chose the city of Indianapolis and Community Westview Hospital not only to complete his residency, but also to establish himself in the community to provide consistent health care for its residents. Dr. Snyder is a true patient advocate as he works closely with each individual to navigate the complexities of the health care system. He enjoys seeing patients of all ages. In early 2018, Dr. Snyder transitioned from his family practice to providing wound care services at the Advanced Wound Center on the Community Hospital East campus and care at Community MedCheck locations.
