Location: The Supported Employment Consultant position is located in Indianapolis, IN. 


Job Duties
  • Recognizes and commits to critical importance of customer satisfaction
  • Provides comprehensive employability assessments and strategic planning for clients
  • Develops individualized treatment plans specific to employment
  • Provides effective and efficient vocational interventions including crisis intervention
  • Develops and facilitates job placements in the community. 
  • Provides job-specific orientation, training and support as needed
  • Maintain relationships with supervisors and co-workers at job sites to monitor progress performance. 
  • Develops and implements effective and efficient transfer and/or discharge plans as appropriate to client needs. 
  • All other assigned duties as needed. 

Experience
  • Education or five (5) years relevant experience; prefer one (1) year experience in a similar position. 

Education
Bachelor’s Degree from an accredited college or university in behavioral science, business, administration, sales or marketing.


License

Current, valid Indiana Driver’s license and proof of auto insurance required and a driving record that adheres to CHI Policy.

  • Must have sales and marketing skills, problem solving abilities/good judgment, interpersonal skills including flexibility, persistence and patience
  • Must have excellent communication skills (written and verbal)
  • Must have excellent assessment and service competencies specific to population being served
  • Ability to operate a motor vehicle according to Network policy, and the ability to provide transportation of clients in personal vehicle
  • Must possess, by specialized training or experience, age-specific competencies to deal with the needs of the patient population served.
