Job Duties
Provides direct patient/family care through bio-psychosocial assessment, treatment plan development, individual, group and family therapy, case management, discharge planning, collateral consultation and comprehensive referral services.  More specific duties below:
  • Contribute to the development, implementation, and management of Transitional Care Management services as an inter-professional model of care to support high risk patients after discharged from acute settings.
  • Develop partnerships with community leaders through relationship development skills so to bridge systematic gaps in healthcare and meet quality assurance measures of transitional care.
  • Collaborate with physicians, family medicine residents and medical staff in both the hospital and clinical setting in order to implement effective patient treatment plans and address psycho-social barriers.
  • Provide ongoing education to staff and family medicine residents on psycho-social issues and behavioral health topics to address patient barriers, mental health, and care coordination.
  • Facilitate mental health therapy in an individual setting in order to address mental health and positive well-being for patients.
  • Supervise masters level social work student which includes interviewing, onboarding, training, and ongoing evaluation of services and education. 

Experience

Two (2) years experience providing mental health treatment preferred.

Education
Master's degree in social work or related field required.

LicenseMust have LSW or higher from the State of Indiana
