Job Location 
The Case Manager position is located in Indianapolis, Indiana.

Job Duties
The Case Manager is responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of services across the continuum of care to ensure quality patient outcomes and appropriate utilization of health care services.
The Case Manager is responsible for supporting the healthcare team towards a smooth transition from one level of care to another in support of the patient/family.

Experience
Master’s Degree with three years of recent clinical experience preferred.
If not Master’s prepared, then three years of recent clinical experience in a health related area required.

Education

Master’s Degree in Social Work preferred.
BSW with three years clinical experience in a healthcare related area may be considered.

License

Currently licensed by the State of Indiana as a Social Worker required.
Certified Case Manager preferred.
