This physician will join Community Physician Network, a multispecialty group with over 1,000 providers in Central Indiana. Highlights
Community Physician Network is seeking a compassionate BE/BC Geriatrician, Family Medicine or Internal Medicine physician to join Touchpoint Senior Living, our comprehensive program caring for geriatric patients in long-term and acute care settings. This is a PRN positionlocated on the north side of Indianapolis, offering:

· Core practice hours Monday-Friday from 8AM-5PM
· Robust, single EMR (EPIC)

About Community Touchpoint: Community Touchpoint is a service for seniors ages 55 and older. The Touchpoint Senior Living practice consists of experienced physicians and nurse practitioners, dedicated to serving the geriatric population in post-acute and long-term care settings. The physician-led provider team works closely with skilled nursing facilities care teams and various senior living care settings to achieve best possible outcomes for elderly patients. Licensure & Certification
BE/BC in Geriatric Medicine, Family Medicine, and/or Internal Medicine and eligible for medical licensure in Indiana.
