This physician will join Community Physician Network, an employed physician group of over 600 physicians in Indianapolis, IN.

Community Physician Network is seeking a Geriatrician or Primary Care Physician with a strong Geriatrics background to join Community Touchpoint, our comprehensive program caring for geriatric patients in both long-term/acute care and clinic settings. This full-time position includes:

· Ability to practice in a variety of settings (consultative clinic and skilled nursing facilities)
· A dedicated and compassionate interdisciplinary team
· Core hours Monday-Friday from 8AM-5PM
· Robust, single EMR (EPIC) 
Community Touchpoint is a service for seniors ages 55 and older. The Touchpoint Senior Living practice consists of experienced physicians and nurse practitioners, dedicated to serving the geriatric population in post-acute and long-term care settings. The physician-led provider team works closely with skilled nursing facilities care teams and various senior living care settings to achieve best possible outcomes for elderly patients. 
Touchpoint HATS is an outpatient clinic for comprehensive assessment of common issues related to aging, such as memory problems or falls. Our team includes a geriatrician (physician specializing in care for aging patients), a social worker and a nurse practitioner. Patients may be self-referred, referred by a loved one or referred by a physician. For more information please visit
Touchpoint HATS provides a wide range of assessments and services, including the following:
• Comprehensive geriatric assessment
• Cognitive assessment with onsite neuropsychological testing available if indicated
• Assessment of caregiver burden
• Depression assessment
• Falls and mobility evaluation
• Functional decline assessment
• Medication review and counseling
• Needs assessment
• Suitability of living arrangement

Licensure & Certification
BE/BC in Geriatric Medicine and eligible for medical licensure in Indiana.
