Community Physician Network is seeking a Family Medicine Physician to join our outpatient practice at the Washington Pavilion near the campus of Community Hospital East on the east side of Indianapolis, IN.
• Outpatient only
• No OB
• Pavilion setting that also houses urgent care, Ob/Gyn, Pediatrics, Rheumatology, PT and Rehab, Imaging, Infusion and lab services
• Shared call schedule
• Patient Centered Medical Home practice model
• Electronic Medical Record (Epic)
Licensure & Certifications
• Board Certified or Board Eligible in Family Medicine
• Licensed in Indiana or eligible for medical licensure in Indiana
