Job Location
The Home Care Occupational Therapist (OT) position is located in Indianapolis, Indiana.

Job Duties
  • The Occupational Therapist case manages the patient's care to ensure that the established quality outcome targets are met and that appropriate visits are made
  • Performs an in-depth occupational therapy evaluation within 48 hours of physician's order for therapy only patients, or as ordered by physicians
  • Interprets the results of the initial evaluation, determines the treatment plan (including utilization of OASIS outcome measurement items) with appropriate patient goals in conjunction with the physician and patient

Home Care/Hospice experience preferred

Graduate of an accredited Occupational Therapy program

Valid Indiana State Occupational Therapy License and CPR required
