This Nurse Practitioner will join Community Health Network, a multispecialty group with over 1,400 providers in Indiana. This opportunity will be a float position dedicated to our West Indianapolis clinic locations, spanning Mann Road, Mooresville, and Avon.
Community Health Network has an excellent opportunity for a Nurse Practitioner to work in our new Community Clinics at Walgreens. Community Health Network's Community Clinics put high quality, convenient medical care throughout all regions of Indianapolis and the surrounding suburbs.
• Clinic hours are Monday - Friday 9:00 am - 7:00 pm; Saturday
and Sunday 9:00 am - 4:00 pm. Every other weekend requirement.

Nurse Practitioner will provide healthcare, including assessment, diagnosis, and treatment of acute and chronic health conditions and wellness preventative medicine. Promotes a healthy lifestyle and involves the client in his/her plan of care with emphasis placed on patient self-determination and maximization of client knowledge to enhance care of oneself.
Minimum three to five years of family medicine or urgent care experience as an advanced practice provider is required; this is an independent position requiring a strong level of autonomy.
Educational background consistent with requirements for licensure and certification with the State of Indiana
