This primary care, family medicine opportunity will be a dedicated float position for three family practice locations in Madison County: Fortville, Lapel, and Pendleton.
Community Health Network has an excellent opportunity for an experienced, board certified nurse practitioner to practice family medicine within our Madison County locations.
• Practice hours are Monday - Friday 7:30 am – 6:00 pm
• Float position for Madison County: Fortville, Lapel, and Pendleton Family Medicine Practice locations
• Autonomy in providing patient care. Busy offices with high patient volume

Minimum 2 to 5 years of nurse practitioner experience in family medicine is firmly required
Nurse practitioner will provide healthcare, including assessment, diagnosis, and treatment of acute and chronic health conditions and wellness preventative medicine. Promotes a healthy lifestyle and involves the client in his/her plan of care with emphasis placed on patient self-determination and maximization of client knowledge to enhance care of oneself
Educational background consistent with requirements for licensure and certification with the State of Indiana
