This nurse practitioner or physician assistant will join Community Health Network, a multispecialty group with over 1,100 providers in Indianapolis, IN. This adult medicine opportunity will be located at our Diabetes and Internal Medicine Care Pavilion at 8205 East 56th Street, Suite 100, Indianapolis, IN 46216.

Highlights
Community Health Network has an excellent opportunity for an adult/gerontology medicine trained nurse practitioner or physician assistant to work at our Internal Medicine practice in NorthEast Indianapolis, with a focus on diabetes and endocrinology.

• Practice hours are Monday - Friday 8:00 am-5:00 pm
• No weekend or on-call requirements
• Electronic Medical Record (EPIC)

Job Duties
Internal Medicine nurse practitioner or physician assistant will provide healthcare, including assessment, diagnosis, and treatment of acute and chronic health conditions and wellness preventative medicine for adult and geriatric patients. Promotes a healthy lifestyle and involves the client in his/her plan of care with emphasis placed on patient self-determination and maximization of client knowledge to enhance care of oneself.

Education

Educational background consistent with requirements for licensure and certification with the State of Indiana

Experience
Minimum three to five years of adult/gerontology medicine or family medicine experience as an advanced practice provider is firmly required
