This Nurse Practitioner will join Community Health Network, a multispecialty group with over 1,200 providers in Indiana. This opportunity will be a float position dedicated to our West Indianapolis Community Clinic at Walgreens locations, inclusive of Avon, Mooresville, and Mann Road clinics.


Community Health Network has an excellent opportunity for a board-certified, family medicine trained Nurse Practitioner to work in our Community Clinics at Walgreens West locations. Community Health Network's Community Clinics put high quality, convenient medical care throughout all regions of Indianapolis and the surrounding suburbs.

• Clinic hours are Monday - Friday 9:00 am - 7:00 pm; Saturday 
  and Sunday 9:00 am - 4:00 pm. Every other weekend requirement
• 0.9 FTE (36 hours/week)
• Electronic Medical Record (EPIC)
• Autonomous model of primary care with support of offsite collaborating physician
• 12 total clinic locations with opportunity to pick up extra shifts

Job Duties
Nurse Practitioner will provide healthcare, including assessment, diagnosis, and treatment of acute and chronic health conditions and wellness preventative medicine. Promotes a healthy lifestyle and involves the client in his/her plan of care with emphasis placed on patient self-determination and maximization of client knowledge to enhance care of oneself.

Minimum two to five years of family medicine or urgent care experience as an advanced practice provider firmly required; this is an independent position requiring a strong level of autonomy

Educational background consistent with requirements for licensure and certification with the State of Indiana

Demonstrate a high level of expert skill in delivering exceptional, quality care while enhancing the well-being of the patient
