This nurse practitioner will join Community Physician Network, a multispecialty group with over 1,100 providers in Indianapolis, IN. Meridian Street, Suite 110, Carmel, IN 46032.
Highlights
Community Health Network is currently seeking a board certified, adult/gerontology medicine trained nurse practitioner for the opportunity to practice Internal Medicine at our Carmel Pavilion location.
• Practice hours are Monday - Friday 7:00 am-5:00 pm
• Outpatient only
• 36 patient contact hours and 4 administrative hours required weekly. Opportunity to consider part-time hours (24 hour/week commitment) as well
• Autonomy in providing patient care, with support of a collaborating physician
• Opportunity to establish lasting rapport and patient relationships
• Electronic Medical Record (EPIC)

Experience
Minimum three to five years of adult medicine experience as a nurse practitioner is firmly required.
Job Duties
Internal Medicine nurse practitioner will provide healthcare, including assessment, diagnosis, and treatment of acute and highly chronic health conditions and wellness preventative medicine for adults.
Education
Educational background consistent with requirements for licensure and certification with the State of Indiana
