This family medicine opportunity will be located on our Alexandria Family Medicine Care practice at: 2116 S. Park Avenue, Alexandria, IN 46001.


Community Health Network has an excellent opportunity for a board certified, family medicine trained nurse practitioner to work at our outpatient established Family Medicine Care practice in Alexandria, IN.

• Full-time position with competitive compensation and benefit package
• Monday through Friday schedule, no call requirement
• Autonomy in providing patient care. Busy office with high patient volume
• Opportunity to establish lasting rapport and patient relationships
• Electronic Medical Record (EPIC)

Minimum 3 to 5 years of nurse practitioner experience in family medicine is firmly required

Job Duties

Nurse practitioner will provide healthcare, including assessment, diagnosis, and treatment of acute and chronic health conditions and wellness preventative medicine. Promotes a healthy lifestyle and involves the client in his/her plan of care with emphasis placed on patient self-determination and maximization of client knowledge to enhance care of oneself.

Educational background consistent with requirements for licensure and certification with the State of Indiana
