Community Physician Network, part of Community Health Network, has an excellent opportunity for an experienced Nurse Practitioner or Physician Assistant to work for our Family Medicine Care, South Indianapolis location. Family Medicine Care provides a patient-focused practice, which delivers evidence-based healthcare and a full range of health care services. Ideal candidate must be flexible in schedule of hours worked based on the access needs of the patient population being served.

Licensure & Certifications
Requires certification and current Indiana RN and APN licensure, as well as CSR prescriptive authority or Physician Assistant state licensure.
1+ years of FM experience as an NP or PA
Educational background consistent with requirements for licensure and certification within the state of Indiana
Demonstrate a high level of expert skill in delivering exceptional quality of care while enhancing the wellness of the patient
