Job Location
This provider will join an employed group of over 900 providers with Community Physician Network. The clinic is located at Family Medicine Care 1601 Medical Arts, Blvd, Anderson, IN.

Community Physician Network, part of Community Health Network, has an excellent opportunity for an experienced or new graduate Nurse Practitioner to work for our Family Medicine Care, Anderson location. Family Medicine Care provides a patient-focused practice, which delivers evidence-based healthcare and a full range of health care services.   • Patient Centered Medical Home practice model
  • Electronic Medical Record (EPIC)
  • Competitive compensation and benefit package
  • No call associated, could be a possibility based on patient. Monday- Friday (Days), later coverage may be needed based on patient volume and needs.
Licensure & Certifications
  • Must have MSN with preparation as an Advance Practice Nurse
  • 2+ years of FM experience as an NP
  • Educational background consistent with requirements for licensure and certification within the state of Indiana
    Skills
  • Demonstrate a high level of expert skill in delivering exceptional quality of care while enhancing the wellness of the patient
  • Requires the ability to integrate research and management to educate patients, families, nursing staff and physicians on medical options and choices of care.
  • Must conduct comprehensive health examinations while autonomously evaluating patients.
