The Life Skills Clinician-School Based is located in Kokomo, Indiana

Job Duties
  • Participates in development of curricula for individual and group services that meet the individual needs of the client/family served.
  • Trains and assists and/or family in the development of living skills, individually or in groups.
  • May assist with gathering data for comprehensive psycho-social assessments. 
  • Monitors individual client/family outcomes as related to skill acquisition and training. 
  • Communicates need to update plan of care as it is related to skill acquisition and training. 
  • Evaluate functioning and identify ongoing risk factors and communicate these to the client/family team. 
  • Participates in the development of individualized treatment plans in collaboration with the client/family team. 
  • Collaborates as an active multidisciplinary team member. 
  • Provides services in homes, school, and/or other community sites
  • When necessary, will transport clients in personal vehicle. 

One (1) year experience in a human services related field involving skills training and/or case management preferred. 

EducationBachelor's degree required (in Mental Health related field preferred) or 8 years equivalent work experience. 

Current Indiana's Driver's license and proof of auto insurance required; driving record that adheres to CHI policy
