Community Health Network is seeking a Registered Nurse (RN) to work as a Diabetes Care Coordinator, serving the patients of Community Heart and Vascular Hospital and Behavioral Health Services North. This is a unique opportunity for a nurse who is passionate about the diabetic patient population to use his/her teaching and customer service skills.

What you will do:
  • Assist the care team in glucose control and management of diabetes related needs for hospitalized patients to achieve optimum outcomes in a cost effective manner.
  • Demonstrate performance consistent with professional standards of practice, care, performance, and the mission and goals of the organization.
  • Coordinate diabetes related care and transfer/discharge planning with multi-disciplinary team members utilizing appropriate resources.
  • Demonstrate compliance with legal aspects of patient care activities, guidelines, policies, and procedures that impact practice.
  • Demonstrate affinity to provide care specialized to the control of hyperglycemia and management of diabetes care.
  • Function as an effective team member as evidenced by interpersonal skills and compliance with group norms and expectations.
  • Accept responsibility and exercise legitimate authority and accountability for practice care management.
What you will need:
  • Nursing degree from ACEN or CCNE accredited school of nursing, ASN or BSN (or three years RN experience)
  • RN licensure from the Indiana Professional Licensing Agency
  • Three years professional nursing experience that includes care of the diabetic patient
  • Certification as a diabetic educator, or is eligible to sit for the exam within one year of successful program completion
